Usually P0401/402 are to do with a bad DPFE sensor. What you can do to test if it really is giving too much flow is pulling the vacuum tube off the top of the egr and feel for suction at idle. There should be none. If there is then you need to replace the EVR valve (EGR vacuum solenoid). 99% of the time the DPFE sensor must be replaced though.

The ccv re-route, a new EGR valve (updated design), the most recent flash update, and fuel cetane booster each tank - now what coats the EGR valve is just a dry powder (as it should be). I would be concerned about the coolant loss.
It is the EGR valve 100%! I had Identical symptoms with my 2011 Ford Kuga Diesel 2.0 litre 6-speed manual box 100,000 miles, making joining fast traffic very risky and overtaking a true nightmare. It all started after a cambelt change but turned out to be red herring and had nothing to do with this- just a coincidence.
